2. Complete the following forms:

   • DCSD Online Admissions Form-click on the plus signs (+) for more info and the admissions online form

   • DCSD Online Notification of Intent to Homeschool Form

Additional Required Forms:

• Intent to Enroll 2026-27

• Media Consent Form: Complete using Adobe Pro or print and fill out by hand.

• Once completed, email both required forms to Lisa Sharpe, Registrar – llsharpe@dcsdk12.org.

Immunization Exemptions

All CDPHE immunization exemption certificates expire on June 30, 2026.
When registering for the 2026–27 school year, you must complete the CDPHE Immunization Exemption process to finalize your Online Admissions form.

Please note: You will need to complete the exemption process again after July 1, 2026, for the 2026–27 school year and submit the new certificate to our nurse.
